A formulary is a term used to describe the list of prescription drugs approved and covered by an individual health insurance plan. Formularies are unique to each plan, and only your health insurance plan controls which medications they include. Some formularies may divide medication into different tiers and specify what conditions they cover as treatment. In many cases, a health plan’s formulary will also include the procedures to limit or restrict some types of medication.
Some health insurance plans call these “drug lists,” “covered medications/drugs,” or “approved/preferred drug lists.”
